Insurance accepted
Clear Dermatology & Aesthetics Center proudly accepts almost all major commercial insurances, including Medicare. However, health insurance companies frequently change; therefore, we encourage all patients to contact their health insurance company directly to ensure that we’re in network before making an appointment. This will also give you an opportunity to review your co-pay requirements to prepare you for any out-of-pocket costs (including deductibles) that you might be responsible for. Additionally, because insurance coverage can change throughout the year, it is important to check with your insurance company for specifics, including whether or not you need a referral or preauthorization for each appointment you schedule. Please note that we may not be in network with all plans for all payers.

In order to best expedite your visit and minimize your waiting time at Clear Dermatology & Aesthetics center, we kindly ask that you complete the “New Patient Registration” and “Medical Questionnaire” prior to your visit if you are a new patient. If you are already an established patient and need to update your medical history, please complete the “Medical Questionnaire” form.
